HAZARDOUS WASTE SURVIVAL GUIDE
Forms & Worksheets

These forms & worksheets are included to provide a structure for collecting and organizing the information needed to evaluate waste reduction opportunities. Additionally, they can be a record of your waste minimization efforts for compliance purposes.

Note for Online Version: Print out the forms & the hardcopy versions for your records. Each form and worksheet is hyperlinked to the primary corresponding discussion in Chapter Three: Your Waste Minimization Program.

Walkthrough Report: Form B

Date: _________________

Use this form to guide your observations during the walkthrough. Follow your operations from delivery of raw materials, through production/service operations, to final waste management methods. Your initial impressions can provide vital clues about how your wastes are generated, and how they might be controlled.

Verify Previously Collected Information

Verify the information collected during the preliminary review and complete the waste profiles (Form A). Look for the following in particular:

o Wastestream characteristics and sources of generation
(Get employee comments about the sources of waste in the facility.)

o Waste management practices (handling, storage, "disposal")
(Describe any associated problems or potential hazards.)

o Waste minimization practices in place (including good "housekeeping")
(Do they appear to be effective?)

Look for Indications of Poor Housekeeping and/or Planning

Y   N

o   o   Cluttered walkways and storage areas

o   o   Unswept or stained floors

o   o   Uncovered/open containers

o   o   Drums stacked improperly, shelving too small to properly handle inventory

o   o   Stockpiling of empty drums/containers

o   o   Lack of or illegible container labels

(For those which are there, do they describe contents and hazards? If "waste," do they have a "hazardous waste" label?)

o   o   Out-dated stock or materials no longer in use are still in storage

o   o   Large quantities of stock on hand of items not used often

o   o   History of spills, leaks, accidents or fires

Look for Indications of Material Losses and/or “Hidden” Sources of Waste Generation

Y   N

o   o   Noticeable spills or leaking containers

o   o   Discoloration or corrosion on walls, work surfaces, ceilings, floors, or pipes

(May indicate system leaks or poorly maintained equipment.)

Look for Indications of Material Losses and/or "Hidden" Waste Generation, cont.

Y   N

o   o   Visible smoke, dirt, or fumes

o   o   Strange odors or eye, nose, or throat irritation when first enter the workplace

o   o   Dripping or running water, steam, evaporation, drag-out, etc.

Look for Necessary Safety Measures

Y   N

o   o   Hazard warning signs are posted and clearly visible

o   o   Emergency equipment available and visible to ensure rapid response to fire, spill, or other incident

o   o   Safety equipment in good condition and being used by employees

o   o   MSDSs readily accessible

Look for Indications of Good/Poor Employee-Management Communication

Y   N

o   o   Supervisors are accessible to employees and encourage their participation

o   o   Employees are satisfied with work environment
